I am familiar with the concept (from other games). However, despite the fact that the term is used twice in the VR section of The Mesh chapter, there is no reference to this mechanic or rules elsewhere. What are the mechanical implications of the term's use?

In any case, if it's included in the Mesh section, they should probably add a line item in the Stressful Experiences table of Actions and Combat on page 39. There's not a specific entry for witnessing a death there, but 1d6 seems like an appropriate value for the normal version of that (like Viewing extreme violence/torture).
