OK, take this for a situation.
PCs A, B & C want to enter a door, which they suspect is under guard by a number of goons NPCs.
Now the goons all delay their actions, waiting for someone to step through the door.
Characters A & B also delay there actions, while character C steps through the door to see the goons.
Who interrupts first ?
According to the rules you can use your delayed action to interrupt another players action, but in the case of a number of individuals all with delayed actions who interrputs who and in what order ??? It can get very confusing !

I would recommend that all interrupt actions trigger in initiative order. So in effect, the person with the highest initiative not only acts first, but interrupts first. This simplifies the whole stack. If initiative has not been rolled yet, have all of the characters that were delaying their actions roll initiative first, in order to figure out which one acts first accordingly. Anyone who does not delay an action acts at the end of that phase (albeit in initiative order amongst them).

I'd say that the first one who says he acts, will act first. And then, everybody else holding an action in their initiative orders.
So if the initiative order is A, B, C and they all hold actions, if B calls his action first and then A and B do that too, I would solve it as B, A, C.

In my opinion if everyone is witholding action, then 3 seconds elapses and re-roll initiative. This can continue until the security forces (goons) reenforcements arrive as I'm sure they were called when the security team began witholding action.
BTW IMHO if the goons do not know for sure that there is someone on the other side of the door, they are not witholding action as they do not know there is an action to withold. They also should not be surprised as they are guarding the door and paying attention for it to open. Initiative should be rolled as soon as the door is opened with neither side getting surprise.

The question was how to resolve a delayed action when multiple characters delay, and someone triggers them all simultaneously.

I thought my first paragraph answered that. If everyone is witholding action then no one is acting and 3 seconds elapses. Everyone then needs to reroll initative. This can go on until someone decides to act or some other factor takes effect. This is how I've run this situation in all the games I've run. My players learn very quickly that inaction <> action and sometimes it's better to be first.
If you have multiple characters witholding action while others are not witholding, then at the end of the pass you walk down through the list of prople that are witholding, either in initiative order or what ever order you as the storyteller choose, and ask them what they are doing, if they don't have an answer skip them and move to the next pass/turn which may mean they lose their ability to act. I know this seems like it may be a bit harsh, but harsh is better than a stand-still in the game waiting on someone to do something. If you have NPCs that are also withholding, it's easiest to make one of them do something because honestly who really cares if they get killed, they are grunts afterall.
I'm sorry if my first response was too short to be clear on how to handle the situation, I'll try to be more verbose in the furure.

I'm sorry I think I now understand where I was unclear. The first characer who was witholding action that calls themselves into action acts first, all others follow in initiative order and withold/delay action is no longer available to anyone until next pass.

Yeah, I judged that to be the correct way to handle it as well. It makes sense to handle it in the pre-determined initiative order, but with all delaying characters acting immediately.
